Know where you stand: projected star rating, weighted measure score, projected quality bonus payment, and every measure against its year-end benchmark with an on-track signal.

Quality gaps closed all year. A MIPS submission you can reproduce.
Two related but separate programs under one roof. HEDIS and Stars gap closure is continuous and lives in a workbench with owners and states. MIPS and MVP reporting is annual and ends in a locked, fingerprinted score snapshot with a printable audit packet.
What Q-Track does
Work the gaps: a five-state lifecycle from workable to plan-closed, bulk assignment for managers, personal queues for specialists, and automatic closure when a qualifying result lands in the data.
File MIPS with confidence: a best-six quality optimizer, retained evidence for improvement activities, an inventory of which source feeds each measure, and a submission checklist that ends in a locked snapshot.
The screens, in the order you would use them
- 1
Performance Metrics
The quality scoreboard: the four headline tiles, then every measure with its weight, year-end benchmark, eligible and meeting counts, rate, star, and how far ahead or behind it is running.
- 2
HEDIS Workbench
The gap queue and its reconciliation states: Total Workable, Open, Captured (pending), Submitted (awaiting), and Plan Closed (reconcile). Managers see the full pool with team load; specialists see their own assignments.
- 3
MIPS
Overview with a forecaster showing the range different Cost outcomes would produce, a Quality optimizer that picks the best six measures, Cost, Improvement Activities with retained evidence, Promoting Interoperability, an Inventory matrix, and Submission with a readiness checklist and a locked, fingerprinted snapshot.
- 4
MVP
Pathways, subgroups, and traditional-versus-MVP scoring side by side all year, so the choice is made on numbers rather than in December.
What the figures count
Every figure on every screen has a definition behind it. These are the ones this module runs on.
- Open Gaps, four ways
- The HEDIS Workbench counts member-and-measure pairs the plan reports non-compliant. Performance Metrics counts the same pairs on star-rated measures only. MIPS counts eligible patients who have not met a measure. Risk IQ counts conditions. They were never meant to match, and every gap page says which one you are looking at.
- Captured (pending) and Submitted (awaiting)
- The two states that exist for the wait between closing a gap and the plan acknowledging it. Nothing is lost while a gap sits there, and re-sending slows reconciliation down.
- MIPS final score
- The categories you control. Cost is computed by CMS after submission and is blank for everyone beforehand, which is why the Overview shows a range instead of pretending to know.
What this module will not let you misread
Adding the two Q-Track gap counts together
They overlap. The star-rated count on Performance Metrics sits inside the workbench's workable set, and the two come from different plan loads.
Reading the MIPS score on screen as the score CMS will give
Cost is computed by CMS after submission. Read what you see as the categories you control.
Chasing gaps in Submitted (awaiting)
The next move belongs to the plan. Duplicate submissions slow reconciliation rather than speeding it up; chase the plan, not the item.
Where the data comes from
Payer gap files reconciled against your own records, plus claims and clinical feeds for the MIPS measures, scored to HEDIS MY2026 and the CMS Quality Payment Program rules for PY2026. Alara Population Health is not itself a Qualified Registry; the locked snapshot is handed to your registry or EHR for filing.
How Alara Healthcare uses it
Alara Healthcare's quality service uses the workbench as the outreach queue, so gap closure runs from January rather than in the fourth quarter, and prepares the MIPS submission packet from the same screens the practice sees.
Quality & HEDIS servicesQ-Track questions
I closed a HEDIS gap this morning and the workbench still shows it as open.
That is expected. The open count is only as current as the last file from the plan, so a gap you closed today stays open until the plan acknowledges it. The states in between exist for exactly this wait: Captured (pending) means the documentation is held, Submitted (awaiting) means it is with the plan.
Why does Risk IQ say thousands of open gaps and Q-Track a few hundred?
Neither is wrong. Open gaps names four different populations: Risk IQ counts conditions awaiting re-documentation, the HEDIS Workbench counts member-and-measure pairs, Performance Metrics counts those on star-rated measures only, and MIPS counts eligible patients who have not met a measure. Different units, different rules, different sources.
Our MIPS final score looks strong. Is that the score CMS will give us?
Not quite. The score shown leaves out Cost, which CMS computes after submission and which nobody can know beforehand. Read it as the categories you control, and use the Overview forecaster for the range different Cost outcomes would produce.
See Q-Track on your own file layout.
Bring a sample payer file to the demo and we will show you where it lands, what it surfaces, and who works it.
